Atsali is about the tallest character in the comic (when they're in human form, anyway). I think Phix is probably taller when she's doing her "sexy librarian" thing and seducing Monica's boss and maybe adult Bia was, but i don't think anyone else is.

Paul did a pin-up of Atsali and Castela a while back - the one where Atsali's t-shirt says "Frankie Says Relax" (which is incorrect - i remember those shirts) - and though Little Flower is leaning slightly against Atsali's leg, not standing bolt upright, she barely stands taller than Atsali's hip.

1) Yes it's a thing in some parts of the Midwest.
2) Depents on the song and skill of the player. Like 'Youen Naru Kizuna no Hibiki' for example.
